Hello Lancers,
We hope this email finds you well. This is a friendly reminder that a yearly physical examination is required in order to be cleared for athletic participation in summer camps, tryouts, team practice and games, and all athletic outings. Please remember that physicals are valid for 365 days from the date of the exam.
La Salle Athletics will offer the opportunity to obtain a physical on campus on Sunday, June 7, 2026, from 9:00 AM to 1:00 PM. No appointment is necessary, but please be sure to arrive by 12:00 PM to guarantee that your child will be seen.
Location: 100 Level of Campus
Parking and Entrance: Lower lot off of Michillinda Ave.
Cost: $30, payable at the time of service via cash or check (made payable to La Salle).
Please click here to view and download a blank 2026-2027 school year Physical Form. To expedite the process, we ask that you please complete and sign the first page of the Physical Form prior to arrival. This form must be signed by a parent prior to the student-athlete obtaining a physical.
All student-athletes are also required to register for athletic clearance. To do so, please visit HomeCampus.com. Please click here for instructions on how to register.

If you opt to have a physical by anyone other than La Salle, please be sure it is signed by a medical professional (MD, DO, or Nurse Practitioner). If you have any questions, please email athletics@lasallehs.org.

A current physical examination is required for all student-athletes prior to participating in summer camps, tryouts, team practices, games, and all athletic activities. Please note that physicals are valid for 365 days from the date of the exam. Physicals dated after June 1st will remain valid for the entire 2026–27 school year.
On-Campus Physical Opportunity
La Salle Athletics will be offering on campus physicals during the spring of 2027. Please pay attention to the weekly parent emails under the Athletic section for dates and times.
To expedite the process, we ask that you download, complete, and sign the first page of the Physical Form prior to arrival. A parent signature is required before the student-athlete can be seen. Please click here to download the 2026-27 physical form.
Please note: If your student-athlete obtains a physical outside of La Salle, it must be signed by a licensed medical professional (MD, DO, or Nurse Practitioner).
